Why today: Two checks at the Commerce City waterline flagged spoil piles too close to the edge and a missing daily inspection. A trench does not have to be deep to bury someone — a cubic yard of soil weighs about as much as a small car.

Before anyone enters:

  1. Competent person inspects first — every day, and again after rain or any change. No sign-off, no entry (OSHA 1926.651).
  2. Protective system at 5 feet or deeper unless it is stable rock: slope it back, bench it, or set a trench box. Pick the control before the crew climbs in, not after.
  3. Keep the spoil pile and equipment back at least 2 feet from the edge.
  4. Plan the way out. A ladder or ramp within 25 feet of every worker in a trench 4 feet or deeper.

On the walk: watch for standing water, cracks or bulging in the wall, and any utility that got exposed. If the wall does not look right, get everyone out and call the competent person.
